An Adjustable Loop is a high-tech medical "noose" made of super-strong fiber. Unlike a fixed-size loop, this one can be pulled tighter after it is placed in the body. It works like a heavy-duty zip-tie for surgery—it goes one way and stays locked, allowing a surgeon to pull a new ligament into a bone tunnel until it is perfectly snug.
Its most common application is in ACL Reconstruction. It acts as the "bridge" between the bone and the new tendon. It is also used in the shoulder to hold the collarbone in place after a dislocation (AC joint repair) and in the ankle to stabilize the two lower leg bones (syndesmosis repair).
The main advantage is Zero-Guesswork. Surgeons don't have to pre-measure and choose a fixed loop size. This eliminates the risk of a loop being too long (which causes a loose joint) or too short (which prevents the graft from entering the bone). It provides a custom fit for every patient's unique anatomy.
Biomechanically, the loop is designed to handle Cyclic Loading. When a patient walks, the knee pulls on the loop thousands of times a day. Our locking weave converts that pulling force into a tightening force within the braid. This ensures the loop maintains its tension ($T$) and prevents graft movement, which is essential for the bone to grow into the tendon.
The loop is typically passed through a bone tunnel. Once the graft is attached to the loop, the surgeon pulls on the "leader" sutures. This action shortens the loop and draws the graft into the tunnel. The surgeon can feel the tension and visually confirm that the graft is fully seated before trimming the excess suture material.
Because this loop provides such high initial stability, patients can often start physical therapy and Early Range of Motion much sooner than with traditional techniques. The material is non-absorbable, meaning it stays in the joint permanently, acting as a lifetime safety belt for the reconstructed ligament.
